Group:
For quote requests, please complete the RFP & Census form (Excel) or (PDF) When you send the completed form, specify which type(s) of coverage you would like quoted and provide the current & renewal rates with the plan design(s) if your company already has a plan(s) in place.
Individual:
The Open Enrollment Period (OEP) for medical coverage is Nov 1st - Dec 15th. If you have a qualifying event, you may be able to enroll outside of this timeframe. Contact me, so we can review your situation. ​

Other individual products such as: dental, vision, accident, hospital indemnity, and cancer plans allow enrollment year round.
Medicare Plans:
-Turning 65
-Qualifying due to Social Security Disability
-Getting ready to retire
-Moved to a new to area and your plan is not offered
-Gaining or losing Medicaid or Low-Income Subsidy (LIS)
-Medicare Supplement – Birthday Rule
-Annual Election Period (AEP) Oct 15th - Dec 7th
-MedAdvantage Open Enrollment Jan 1st - March 31st
